Intake Process

Our patient intake process ensures a smooth transition into treatment. The process officially begins when we receive information to schedule an appointment and the patient shows up for the appointment. The steps that follow are the same for any new patient:

Psychiatry Intake Process

Registration

Gathering relevant information regarding your history and completion of intake packet prior seeing the provider.

Vital Signs

Measurement of your blood pressure, heart rate, respiration rate, temperature, oxygen level and other indicators of your body’s vital functions.

Assessment

Systematic collection and analysis of health-related information to identify and support beneficial health behaviors and shifts from potentially harmful health behaviors.

Determination

The psychiatrist decides on a course of action—communicating the plan to be followed and helping you to take the right measures to maintain or manage your health.

Prescription

Psychiatric counseling is recommended in addition to a psychotropic medications regime to treat mental illnesses.

Primary Care Intake Process

Check-In

New patients sign in with our front desk staff and begin the intake process. Consent forms, tests, and questionnaires are filled out and returned to the intake coordinator.

Testing

Vital signs are obtained. Vaccination status is checked or updated. Lab tests including complete blood count, chemistry panel, and urinalysis are ordered.

Examination

Information about you and your health is obtained by observing and talking with you. This includes your memory, mental quickness, skin appearance, heart, ability to stand, etc.

Treatment

In addition to diagnosis and treating acute and chronic conditions, the family medicine provider may offer routine health screenings, preventive care, and counseling on lifestyle changes.

Results

Communication with laboratories and review of electronic health records help manage test results and facilitate patient notification. Follow up for abnormal results next.

We offer the option to schedule appointments online for convenience and to give you the intake process.

E-Forms

With online intake forms, you can instantly upload information and access your electronic health record. This reduces errors and saves time.

Digital Communication

We can implement text and email communication between you and us as well as send alerts via email, phone, or text.

Patient Access Portal

You can use a portal to access information, make payments, schedule appointments, and communicate with the office.
